Damaged / Duplicate or Lost Passport

Documents required:

• Duplicate passport application form which is same as new passport application

• Four passport size photographs (background white)

• The police certificate(FIR) and receipt (NCR)

• Address proof (old passport photocopy if available)

• Date of birth proof

• Notary for loss of passport (Annexure I / lost affidavit )

 

Mandatory documents:

• Annexure L (Affidavit for lost/damaged passport)

• Old passport (if available)

• Police FIR (if damaged beyond recognition)

 

Birth Proof (Any 1):

• Birth certificate (Mandatory for the applicant born on or after 26/01/1989)

• School leaving certificate

 

Address Proof (Any 2):

• Aadhaar card

• Voter ID

• Latest Electricity bill

• Latest Landline/Mobile bill

• Latest Gas bill

• Latest Water bill

• Photo Passbook

• Income tax assessment order

• Spouse's passport copy
