• Address proof (voter id card / bank account / rashan card ) Photocopy

• Date of birth proof (school certificate) Photocopy

• VC (verification certificate) (only for tatkal scheme)

 

Old Passport - If renewal (validity expired or due to expire in a year)

 

Address change (Any 3):

• Aadhaar card

• Voter ID

• Latest electricity bill

• Latest landline/mobile bill

• Latest gas bill

• Latest water bill

• Photo passbook

• Income tax assessment order

• Spouse's passport copy

 

Name change:

• Marriage certificate/ Annexure D (For Change in Name of Female, after Marriage)

• Annexure E (Affidavit for Change in Name/Deed poll/Sworn) & newspaper ads in 1 local and 1 national newspaper (if general name change)
